For Participants: I have carefully considered my decision, the benefits and risks involved and hereby give my informed consent to use the Center and/or participate in the Programs. I have read and understand this Waiver and Release, any questions of mine have been answered, and I voluntarily agree to the above provisions. It is my intent to bind my heirs, next of kin, assigns and legal representative. I HAVE CAREFULLY READ THIS AGREEMENT AND I FULLY UNDERSTAND ITS CONTENTS. I AM SIGNING THIS DOCUMENT OF MY OWN FREE WILL. *

For Parent(s)/Guardian(s): I have carefully considered my decision, the benefits and risks involved and hereby give my informed consent, on behalf of the below listed minor child, for the child to use the Center and/or participate in the Programs as set forth in the above Waiver and Release, and such terms are incorporated herein. I have read and understand the above Waiver and Release, any questions of mine have been answered, and I voluntarily agree to all such provisions. It is my intent to bind my and the minor Participant’s heirs, next of kin, assigns, and legal representatives. I HAVE CAREFULLY READ THIS AGREEMENT AND I FULLY UNDERSTAND ITS CONTENTS. I AM SIGNING THIS DOCUMENT OF MY OWN FREE WILL. *

Release and Waiver: In consideration of and in order to be allowed to participate in lessons/camps, Participant/Parent/Guardian do/does hereby release and forever discharge and hold harmless the College and its successors and assigns from any and all liability, claims, demands, costs and damages of any kind, whether arising from tort, contract or otherwise, which Participant their heirs, assigns, next of kin or legal representatives may have or which may hereinafter accrue, arise from, or are in any way related to Participant’s participation in the lessons/camps, including but not limited to personal injury, bodily injury, illness, property damage, property loss, loss or death, whether caused wholly or in part by the simple negligence, fault or other misconduct of the College and its successors and assigns, other than their intentional or grossly negligent conduct. Participant/Parent/Guardian understands that there are certain inherent risks related to Participant’s use of the Center and/or participation in the lessons/camps. Participant/Parent/Guardian has/have full knowledge and understanding of the inherent risks associated with any activity and/or lesson/camp participation, including but in no way limited to: (1) slips, trips, and falls; (2) exercise and exercise equipment injuries; (3) aquatic injuries; (4) athletic injuries; and (5) illness, including exposure to and infection with viruses or bacteria. Participant/Parent/Guardian further acknowledges that the preceding list is not inclusive of all possible risks associated with any lesson/camp participation and that said list in no way limits the operation of this Waiver and Release. Participant/Parent/Guardian understands and acknowledges that by signing this Waiver and Release Participant/Parent/Guardian knowingly assumes the risk of injury, harm, damage and loss associated with participation in the lessons/camps. Participant/Parent/Guardian also understands that the College does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance in the event of injury, illness, death, property damage, or property loss. *

I HAVE READ, UNDERSTAND, AND AGREE TO COMPLY WITH THE WAIVER AND RELEASE OF LIABILITY AS OUTLINED ABOVE.
