The Family Educational Rights and Privacy Act (FERPA) require Chesapeake College to release non-directory information only to the student. The student may, however, voluntarily waive the privacy rights and authorize certain individuals to receive the non-directory information.
By signing this form, the student waives these rights and allows the following information, when necessary, to be provided to schools for recruitment purposes or to which you are transferring and to media, the NJCAA Region 20, and the National Junior College Athletic Association (NJCAA) as well as any other affiliated organization related to NJCAA athletics:
• Physical/Medical Records
• Grades/Unofficial Transcripts
• Tracers/Transfer Waivers
• Student Academic Progress/Reports
• Financial Records including the Student Air Report (SAR or ISIR)
• Game Film
• Athletic Performance/Stats
• Photos/Media Release
• Release Information
• I further grant permission to any Chesapeake College registrar to release an Unofficial Copy (student copy) of my Chesapeake College transcript to any college Athletic Department.
I authorize Chesapeake College Athletic Department, Coaches, and/or Athletic Training Staff to disclose some or all of the above information to other schools wanting to recruit me.
